Rates and Fees

  

**Initial Evaluation (First Session): ** $250 

**30-Minute Follow-Up Appointments: ** $150 

*All major credit cards accepted for payment. *

  

** Credit Card & Payment Policy

A valid credit card is required to be placed on file before scheduling any appointments.

The card on file will be used for:

  •    Processing late cancellation and no-show fees
  •  Collecting unpaid copayments or outstanding balances

*Copayments must be made before the scheduled appointment. Failure to do so may result in cancellation of the appointment.

*All outstanding fees must be paid before booking or attending future appointments.

*For patients with standing or recurring appointments, any missed session or unpaid balance

must be settled before the next visit.

** Appointment Confirmation & Cancellations

  • Appointments must be confirmed at least 24 hours in advance. Unconfirmed appointments

may be subject to cancellation.

  • Appointments canceled with less than 24 hours’ notice are considered late cancellations.


**No-Show & Late Cancellation Fees

Initial Appointment No-Show Fee: $50

Follow-Up Appointment No-Show Fee: $30

These fees are not covered by insurance and will be automatically charged to the credit card on

file.


**No-Show Fee Waiver Policy

A no-show fee may be waived under the following conditions:

  • You reschedule your appointment at least 24 hours in advance, and
  • You successfully rebook the appointment on the same day, subject to availability.

Please note that same-day rebooking is not guaranteed and is based on provider availability.

****Appointment and Prescription Policy:

  • After initiating stimulant treatment, most patients will have monthly follow-ups.
  • Once stable, up to a three-month supply may be prescribed (in separate 30-day prescriptions or as a 90-day supply, if eligible).
  • Qualification for a 90-day supply is determined by clinical criteria set by your provider.
  • Any request for a new prescription, dose change, formulation change, or additional medication requires an appointment.

 


Termination of Services:


Termination of services is a clinical decision made with careful consideration of the client’s best interests, safety, and the ethical standards of psychiatric-mental health practice. The following are circumstances in which termination may be indicated:

  • Lack of Benefit or Potential Harm:
    Services may be terminated if it is determined that the client is no longer benefiting from treatment, is unlikely to benefit, or if continued therapy may result in harm. This decision is based on ongoing clinical assessment and documentation.
  • Ethical Concerns or Boundary Violations:
    Termination may occur in cases of ethical conflicts, dual relationships, or boundary violations that compromise therapeutic objectivity or professional standards.
  • Client Threats or Abuse:
    If a client engages in threatening, abusive, or violent behavior toward the provider or others within the therapeutic setting, services may be terminated to ensure the safety of all parties.
  • Therapist’s Inability to Provide Care:
    Circumstances such as illness, burnout, or changes in provider availability may necessitate the discontinuation of services. In such cases, reasonable notice will be provided whenever possible.
  • Client Needs Beyond Provider’s Expertise:
    If the client requires a higher level of care or specialized treatment outside the scope of the provider’s practice, a referral to an appropriate resource will be offered.

Procedure:

  • Clients will be informed of the reason for termination in a timely and respectful manner.
  • When appropriate, referrals to alternative providers or resources will be provided to support continuity of care.
  • Documentation of the rationale for termination and any referrals made will be maintained in the client’s record.
